Assessment & Treatment
The client will always be assessed by a qualified physiotherapist.
Our physiotherapists are all registered with the Healthcare Professions Council
and Chartered Society of Physiotherapy and will examine and treat the client in
line with the Chartered Society of Physiotherapy code of practice. After examination
the physiotherapist will explain their assessment of any concerns and propose
treatment if appropriate, explaining what is involved, including any risks. The
client may decline any of the treatments proposed, without prejudice.
The client’s consultation and treatment will usually
be on a one-to-one basis (unless otherwise agreed) and in the privacy of the
treatment room/cubicle or in a chosen domiciliary setting.
The client’s appointment/s will be scheduled
according to their needs and to the best of our ability.
Anyone may book an appointment and this can be done
by telephone or email.
Standard initial assessments are one hour long
consisting of 50 minutes treatment and 10 mins of notes.
Standard follow-on appointments are 45 minutes long
consisting of 35 minutes treatment and 10 minutes of notes.
Requested reports and/or letters are charged in
respect of the total time taken to prepare them. They are based on the standard
therapy costs as shown above.
Telephone conversations over 15 minutes will be
charged and are again based on the above standard therapy costs.
Fit4LifePhysio
requires 24 hours-notice of any cancellation.
If
the client needs to cancel / re-arrange any appointments, please give us 24hrs
notice. The ways to can contact us to advise us of a cancellation are by
telephone by email. Out of normal working hours there is an answerphone to
leave a message.
We
are aware that due to the nature of our service some individuals will have to
cancel with short notice, however we must treat all clients the same and with
less than 24-hours-notice we are unlikely to fill that appointment slot.
Therefore, failure to provide 24 hours-notice of a cancellation for any reason
may incur a cancellation charge which is the full cost of consultation charges.
If
there are a significant number of cancellations made on the clients account, we
will make contact to discuss and agree a solution.
If a patient is more than 15 minutes late for their
scheduled appointment, it will be at the discretion of the therapist if they
can be treated or not.
Payment for all treatments is required at time of treatment
or within 7 days.
We confirm that Fit4LifePhysio hold the appropriate
professional indemnity insurance in respect of the client’s treatment. We
confirm that our Physiotherapists are regulated by the Healthcare Professions
Council, and are members of the Chartered Society of Physiotherapy.
We will treat all information, facts, matters, documents
and all other materials of a confidential nature which we receive or create as
a result of the client’s treatment as confidential. All written, audio, visual
and video records will be stored securely. Information shared within the
treatment sessions will be treated with the strictest of confidence and at no
time will this information be shared with any third parties without consent.
Fit4LifePhysio confirm that we are GDPR compliant and are registered with the
Information Commissioners Office on the “Register of Data Controllers” as
required by the Data Protection Act 1998. All data held by us is processed in
accordance with GDPR.
Clients are required to consent to assessment and
treatment. They are expected to respect the clinician during the treatment
sessions. Aggressive and/or abusive behaviour will not be tolerated. Additional
permission will be required for audio / video recording and for the circulation
of reports to other relevant professionals.
The client is free to withdraw from therapy at any
time without stating a reason provided that 24hrs notice is given for any
booked appointments.
Fit4LifePhysio reserves
the right to terminate therapy at any time.
Fit4LifePhysio reserves the right to amend these terms and
conditions from time to time.
